Scope note: This guide focuses primarily on low testosterone in adult men. Testosterone interpretation, diagnosis, and treatment in women follow different clinical frameworks.
Low testosterone is not diagnosed from one bad blood test.
A proper diagnosis of male hypogonadism requires symptoms or signs that fit testosterone deficiency plus consistently low testosterone measured accurately on more than one occasion. The Endocrine Society's July 2026 statement reinforces the need for at least two early-morning, fasting testosterone tests before diagnosing hypogonadism, with a common clinical threshold near 300 ng/dL.
But 300 ng/dL is not a magic cliff.
Assay quality, sex hormone-binding globulin (SHBG), free testosterone, current health, medications, obesity, age, and most importantly, the reason testosterone is low can change how a result should be interpreted.
I'm not a physician. My perspective comes from years working around longevity clinics, biomarker testing, patient acquisition, and the systems clinics use to move people from an abnormal result to an actual care plan.
That makes me particularly interested in one question:
Did the process establish what is wrong before deciding what to prescribe?
Testosterone replacement therapy can be legitimate medical treatment for appropriately diagnosed hypogonadism.
The problem is when the process starts with:
“How much testosterone should we give you?”
instead of:
“Why is your testosterone low?”
A testosterone number is data.
Hypogonadism is a diagnosis.
Confusing the two is how people can end up treating the wrong problem.
Before You Consider TRT: 5 Things to Confirm First
Here is the short version:
- One low testosterone result does not establish hypogonadism.
- Diagnosis generally requires at least two separate early-morning, fasting testosterone measurements plus compatible symptoms or signs.
- Total testosterone is usually the starting test; free testosterone becomes more useful when total T is borderline, or SHBG may be altering the picture.
- LH, FSH, and targeted testing can help explain why testosterone is low.
- If current or future fertility matters, discuss it before starting testosterone.
Those points are consistent with current Endocrine Society diagnostic guidance and AUA/ASRM fertility guidance.
That's the framework for everything that follows:
Confirm the signal. Explain the cause. Then decide what, if anything, deserves treatment.
What Is Low Testosterone, and What Actually Counts as Low?
Clinically, male hypogonadism is a syndrome in which compatible symptoms or signs occur alongside testosterone concentrations that are consistently and accurately low.
The laboratory result matters.
But the laboratory result alone is not the diagnosis.
Why one low testosterone result is not enough
Testosterone varies.
Levels can change from one day to another and across the day. Acute illness, certain medications, nutrition, SHBG abnormalities, obesity, and differences between laboratory methods can also affect either the measurement or how it should be interpreted.
That is why guidelines recommend confirming an initially low testosterone result rather than turning one measurement into a lifelong diagnosis.
A testosterone result of 278 ng/dL drawn under poor testing conditions tells you something.
It does not tell you everything.
What is a normal testosterone level for your age?
There is no single age chart that defines the correct testosterone level for every man.
A major harmonization study involving 9,054 men established a reference interval of 264–916 ng/dL in healthy, nonobese men ages 19–39 after measurements were standardized to a CDC reference method.
That is useful laboratory context.
It is not a universal treatment target for every 42-, 57-, or 68-year-old man.
Testosterone tends to decline with age, but current Endocrine Society guidance emphasizes that the basic definition of hypogonadism does not change just because a man gets older: symptoms or signs and accurately confirmed low testosterone still matter.
So if someone says:
“You're 55, so 320 is automatically fine,”
that is incomplete.
If someone else says:
“You're 55, so you need to be at 900,”
that is incomplete too.
Is there an “optimal” testosterone level?
No universally proven testosterone number is optimal for every man.
“Normal” is tied to reference populations, laboratory methods, and clinical context.
“Optimal” is often used much more loosely.
For men with properly diagnosed hypogonadism who are treated, the goal is generally to restore testosterone into a physiologic range, improve clinically meaningful symptoms, and monitor for adverse effects, not simply push the result as high as possible. The Endocrine Society's treatment guidance has generally targeted the mid-normal range during therapy.
Your best number is not automatically the highest number.
The better outcome is feeling and functioning well without creating new problems in the process.
Is testosterone below 300 ng/dL always considered low?
No.
A value around 300 ng/dL is a commonly used clinical threshold. The Endocrine Society referenced a common threshold near 300 ng/dL in its July 2026 statement, and AUA guidance also uses 300 ng/dL as an important diagnostic cutoff.
But testosterone does not become a completely different biological substance at 299 versus 301.
The better questions are:
- Is testosterone repeatedly low?
- Were the tests done under appropriate conditions?
- Do the symptoms or signs fit?
- Does SHBG or free testosterone change the interpretation?
- What does the rest of the hormonal evaluation suggest?
What is a dangerously low testosterone level?
There is no universal testosterone number that automatically means “medical emergency.”
Very low testosterone can, however, change what deserves investigation.
The Endocrine Society guideline recommends considering pituitary imaging in selected men with severe secondary hypogonadism such as testosterone below roughly 150 ng/dL, particularly when accompanied by persistent hyperprolactinemia, other pituitary abnormalities, or symptoms of a pituitary mass such as new headaches or visual changes.
That does not mean 149 ng/dL is universally dangerous and 151 is universally safe.
It means a very low value may be a clue that the investigation needs to move upstream.
Do This Next: If you have one unexpectedly low testosterone result, do not begin by assuming you need TRT. Ask whether the result needs to be confirmed correctly first.
Which Low Testosterone Symptoms Matter Most?
The symptoms most suggestive of testosterone deficiency tend to involve sexual and reproductive function.
Fatigue, low motivation, reduced concentration, mood changes, and poor exercise performance may matter, but they are much less specific because many other conditions can cause the same symptoms. Endocrine Society guidance specifically warns against diagnosing hypogonadism from symptoms alone.
Sexual and reproductive symptoms
Symptoms and signs that may deserve evaluation include:
- Lower sexual desire
- Fewer spontaneous or morning erections
- Erectile dysfunction
- Reduced sexual activity
- Fertility problems
- In more severe or prolonged cases, loss of body hair, changes in body composition, anemia, reduced bone density, or other physical findings
In the European Male Ageing Study discussed in the Endocrine Society guideline, sexual symptoms, particularly poor morning erections, reduced libido, and erectile dysfunction, had a stronger syndromic relationship with low testosterone than many nonspecific symptoms.
That makes sexual symptoms useful clinical information.
It does not make them a home testosterone test.
Erectile dysfunction, for example, can also involve vascular disease, diabetes, medications, neurological conditions, psychological factors, relationship factors, and other causes.
Fatigue, brain fog, and poor training are not specific
Feeling tired does not mean you need testosterone.
Neither does having a bad month in the gym, sleeping poorly, gaining body fat, or feeling less motivated.
Those symptoms can occur alongside low testosterone.
They can also occur with sleep disorders, calorie restriction, depression, thyroid disease, metabolic disease, medication effects, overtraining, acute illness, and normal life stress.
The symptom should make you curious.
It should not make the diagnosis for you.
Can low testosterone cause depression, anxiety, or fatigue?
Low testosterone can coexist with low mood, fatigue, and related symptoms, but the relationship is not specific enough to diagnose testosterone deficiency from those symptoms.
A systematic review and meta-analysis of randomized placebo-controlled trials in hypogonadal men found small improvements in sexual desire, erectile function, and sexual satisfaction with TRT but no overall improvement in energy or mood.
Other analyses have produced somewhat different estimates for mood-related outcomes, which is another reason not to treat a nonspecific symptom as proof of low testosterone.
Do not dismiss fatigue or mood changes.
Just do not assign testosterone as the cause before doing the diagnostic work.
What does low testosterone look like in younger men?
A low result in a younger man deserves more investigation than “you must be aging early.”
Possible causes can include:
- Testicular disease, injury, or prior treatment
- Hypothalamic or pituitary disease
- Genetic conditions
- Obesity and metabolic disease
- Severe illness
- Certain medications
- Severe nutritional or energy restriction
- Previous anabolic-androgenic steroid use
- Other causes of primary or secondary hypogonadism
Endocrine Society guidance separates primary testicular hypogonadism from secondary hypothalamic-pituitary hypogonadism and recommends investigating the cause once hypogonadism is established.
For younger men, fertility also moves much higher on the priority list.
Can low testosterone affect sperm count or fertility?
Yes, depending on the cause.
But serum testosterone and sperm count are not interchangeable measurements.
Some conditions can impair both testosterone production and sperm production. A man can therefore have low testosterone and impaired fertility at the same time.
You cannot, however, look at one serum testosterone number and accurately predict sperm count.
When fertility is a concern, evaluation may include semen analysis and reproductive hormone testing in addition to the testosterone workup. AUA/ASRM guidance recommends hormonal evaluation in selected infertile men and specifically cautions against exogenous testosterone for men interested in current or future fertility.
How Should Low Testosterone Be Tested Correctly?
The cleanest diagnostic starting point is straightforward:
Measure morning total testosterone correctly, then repeat an abnormal result on a separate day.
Not a symptom quiz.
Not how motivated you felt during your last workout.
Not whether your beard is growing differently.
Why should testosterone be tested in the morning?
Testosterone has a daily rhythm, particularly in younger men, and levels are generally higher earlier in the day.
Current Endocrine Society guidance recommends early-morning, fasting testosterone testing and at least two separate measurements before diagnosis.
That means a morning result and a late-afternoon result are not necessarily comparable enough to treat as interchangeable evidence.
Why do fasting and current health matter?
Current Endocrine Society guidance specifies fasting morning testing and recommends ruling out reversible or transient contributors before labeling a man hypogonadal. Acute illness and medications such as opioids or corticosteroids can suppress testosterone.
If your physiology is temporarily abnormal, your laboratory result may be temporarily abnormal too.
Why do you usually need two testosterone measurements?
Because biological and measurement variability are real.
Repeating an appropriately collected test reduces the chance that a transient result becomes a permanent diagnosis.
That is particularly important when the potential therapy can affect fertility, hematocrit, blood pressure, and the body's own hypothalamic-pituitary-gonadal axis.
What can temporarily make a testosterone result look lower?
Factors that can affect testosterone measurements or interpretation include:
- Testing later in the day
- Acute illness
- Certain medications, including opioids and corticosteroids
- Severe nutritional or energy restriction
- Obesity and metabolic disease
- Conditions that alter SHBG
- Laboratory and assay differences
Not every factor affects every man in the same way.
The point is to avoid diagnosing a chronic hormonal disorder from a result obtained in a temporary or poorly controlled context.
The laboratory method matters more than most people realize
A testosterone result looks extremely precise.
The measurement system is not always equally precise.
The CDC operates the Hormone Standardization Program, or HoSt, to improve accuracy and standardization of serum testosterone testing. As of May 2026, CDC-listed testosterone methods are evaluated against its reference method, with certification based on defined analytical-performance criteria.
The Endocrine Society has also emphasized that nonstandardized assays can produce meaningfully different results from the same underlying sample.
So when you hear:
“My testosterone is exactly 302,”
remember that the number still lives inside a measurement system.
Total Testosterone vs. Free Testosterone: Which Matters?
Total testosterone is generally the first-line test.
Free testosterone becomes particularly useful when total testosterone is near the lower end of the reference range or when SHBG is likely to make total testosterone misleading.
Most testosterone in blood is bound to proteins, especially SHBG and albumin. Only a small fraction is unbound.
That matters because SHBG can change the relationship between total and free testosterone.
Conditions associated with lower SHBG, including obesity and type 2 diabetes, can lower total testosterone even when free testosterone is less affected. Conditions associated with higher SHBG can make total testosterone look more reassuring while free testosterone is lower.
When should free testosterone be measured?
The Endocrine Society recommends considering free testosterone when:
- Total testosterone is near the lower limit of normal
- A condition that alters SHBG is present
When measured directly, equilibrium dialysis is the reference method. When calculated, the calculation should use accurate total testosterone, SHBG, and albumin measurements. The guideline specifically discourages direct analog-free testosterone immunoassays because of accuracy problems.
What is a normal free testosterone level?
This is where a universal internet number becomes dangerous.
Free-testosterone ranges depend heavily on the measurement method.
A standardized equilibrium-dialysis study reported a reference interval of approximately 66–309 pg/mL among healthy, nonobese adult men and 120–368 pg/mL among healthy, nonobese men ages 19–39. The authors cautioned that those ranges still require validation in broader clinical populations.
A newer 2026 study has also proposed age-stratified ranges using directly measured free testosterone, reinforcing the broader point that method and reference population matter.
Do not take a free-testosterone range from a random website and apply it blindly to a result generated using another method.
Interpret the laboratory's validated result alongside:
- Total testosterone
- SHBG
- Albumin where relevant
- Symptoms and signs
- The broader clinical picture
Can you test testosterone at home?
Home testing can be useful as a screening tool, but an at-home result should not become a diagnosis by itself.
The guideline-based diagnostic pathway is built around accurately measured serum testosterone obtained under appropriate conditions and confirmed on another morning. CDC's testosterone standardization work is also based on serum testing.
My practical rule:
If a home test is what gets you to investigate the issue, fine.
If the result is low, confirm it through a properly timed clinical blood test before making treatment decisions.
And no, you cannot reliably diagnose testosterone deficiency by looking at your beard, muscle mass, mood, or libido.
Same total testosterone, different story
Imagine two men both get a total testosterone result of 285 ng/dL.
Man A
He tested late in the day while acutely ill. He has not repeated the test.
Man B
He has persistent compatible symptoms, two properly collected fasting morning tests in the same low range, and additional testing supports hypogonadism.
Same number.
Very different diagnostic confidence.
Now add SHBG, free testosterone, LH, and FSH, and the stories can diverge again.
Do This Next: When you see a low testosterone result, ask four questions: When was it drawn? Was I fasting and otherwise in my usual state of health? Has it been repeated? Could SHBG or free testosterone change the interpretation?
Once the low result is real, the job changes from proving it exists to explaining why it exists.
What Labs Should You Get Before Considering TRT?
There is no intelligent universal “TRT panel” where every man automatically needs dozens of biomarkers.
There is a sequence.
First confirm low testosterone.
Then determine where the problem may originate.
Then investigate plausible causes.
Then establish an appropriate safety baseline before treatment.
Core confirmation tests
For many men being evaluated for suspected hypogonadism, the diagnostic process may involve:
- Morning total testosterone on at least two separate days
- SHBG when interpretation may be affected
- Albumin when calculated free testosterone is needed
- Free testosterone when total T is borderline, or SHBG is abnormal
Not everyone needs free testosterone.
But total testosterone does not tell the entire story in every patient.
Tests that help find the cause
Once hypogonadism has been established, LH and FSH help distinguish primary testicular dysfunction from secondary hypothalamic or pituitary causes.
Depending on the pattern, medical history, and symptoms, additional testing may include:
- Prolactin
- Iron saturation or ferritin
- Other pituitary hormones
- Pituitary imaging in selected cases
- Genetic evaluation in specific presentations
The Endocrine Society specifically recommends prolactin and iron-saturation testing in men with secondary hypogonadism, with pituitary testing or imaging added in appropriate cases.
What do LH and FSH mean when testosterone is low?

That distinction matters because treating the testosterone number does not automatically treat the reason it became low.
Questions and baseline assessments that matter before treatment
Before TRT, a clinician may also evaluate:
- CBC and hematocrit
- Blood pressure
- Prostate risk and PSA when clinically appropriate
- Fertility plans
- Semen analysis if fertility is an active concern
- Sleep-apnea history
- Cardiovascular history
- Current medications
- Previous anabolic-steroid or testosterone exposure
Current Endocrine Society guidance emphasizes baseline hematocrit, appropriate prostate-risk assessment, contraindications, and ongoing monitoring. FDA labeling changes in 2025 also added or strengthened class-wide blood-pressure warnings for testosterone products.
Here is the simpler way to think about it:

The goal of a good hormone workup is not to qualify you for TRT.
It is to explain why your testosterone is low.
Do This Next: Ask your clinician: “What are we doing to confirm the diagnosis, identify the cause, and establish a safe baseline?”
If the entire answer is:
“Your testosterone is 287,”
the diagnostic work is not finished.
What Causes Low Testosterone in Younger and Older Men?
Low testosterone can result from a problem in the testes, impaired signaling from the hypothalamus or pituitary, or potentially reversible suppression from another condition or exposure.
Primary vs. secondary hypogonadism
Think of testosterone production as a chain of command.
The hypothalamus and pituitary signal downstream.
LH plays a major role in stimulating testosterone production in the testes.
FSH is important for spermatogenesis.
Primary hypogonadism means the testes themselves are not responding adequately.
Secondary hypogonadism means the upstream hypothalamic-pituitary signal is impaired.
LH and FSH help distinguish those patterns.
Testicular causes
Potential causes of primary testicular dysfunction include:
- Genetic conditions
- Testicular injury
- Some infections
- Chemotherapy or radiation
- Other forms of testicular disease or dysfunction
Specific presentations may warrant urologic, endocrine, reproductive, or genetic evaluation.
Pituitary and hypothalamic causes
Pituitary tumors and other hypothalamic-pituitary disorders are less common than many everyday contributors to low testosterone.
They are also one reason that:
“Low T = testosterone prescription”
can be a bad shortcut.
Very low testosterone in a secondary pattern, persistent high prolactin, other pituitary-hormone abnormalities, new headaches, or visual symptoms can justify deeper evaluation.
Potentially reversible contributors
Conditions or exposures that can contribute to low testosterone or complicate its interpretation include:
- Obesity
- Type 2 diabetes
- Acute or chronic illness
- Opioid use
- Glucocorticoid use
- Previous anabolic-androgenic steroid use
- Severe nutritional restriction
- Excessive exercise or energy deficit in the wrong recovery context
- Conditions that alter SHBG
The Endocrine Society's July 2026 statement specifically emphasizes ruling out reversible contributors such as obesity and medications before deciding that TRT is the answer.
That is why the cause matters.
If another problem is suppressing testosterone, simply raising the number may leave the original problem untouched.
Can You Increase Testosterone Naturally?
Sometimes.
Lifestyle changes are most useful when they address factors that are actually contributing to low testosterone.
They are much less likely to correct organic testicular or pituitary disease.
When improving body composition can help.
Obesity is strongly associated with changes in total testosterone and SHBG.
Weight reduction can increase testosterone in many men with obesity. A meta-analysis of diet- and bariatric-surgery-based weight loss found increases in testosterone after weight reduction, with the magnitude influenced by the amount and method of weight loss.
The Endocrine Society's July 2026 statement goes further: for appropriately diagnosed hypogonadism associated with overweight or obesity and no other identified cause, weight loss is typically considered first-line therapy.
That does not mean “lose weight” is the answer to every low-T case.
It means metabolic health can be part of the hormonal diagnosis.
Sleep deserves attention
Poor sleep, fatigue, low libido, and impaired recovery can overlap with symptoms people attribute to low testosterone.
More importantly, untreated severe obstructive sleep apnea is specifically relevant when considering testosterone therapy, and established Endocrine Society guidance recommends against starting testosterone in men with untreated severe sleep apnea.
If sleep is chronically poor or sleep apnea is suspected, investigate that as part of the clinical picture instead of assuming testosterone is the only problem.
Training is good. Chronic under-recovery is not.
Resistance training and aerobic exercise are good for health.
But severe energy restriction, excessive exercise, and inadequate recovery can suppress reproductive signaling in some contexts. Endocrine guidance recognizes nutritional deficiency and excessive exercise among potentially reversible causes of secondary hypogonadism.
The goal is not to stop training.
It is to make sure the training, nutrition, and recovery picture makes physiological sense.
What foods lower testosterone?
There is no serious clinical guideline listing individual everyday foods that universally “kill testosterone.”
Overall energy balance, body composition, nutrient adequacy, illness, medications, and metabolic health are usually more clinically meaningful than fearmongering about one ingredient.
Severe calorie restriction can suppress reproductive hormones in some men, while appropriate weight reduction in men with obesity may improve testosterone.
Context matters.
What about testosterone boosters, vitamins, and herbs?
Be skeptical of products promising dramatic testosterone increases without first establishing why testosterone is low.
Correcting a documented nutritional deficiency is different from taking a supplement marketed as hormone replacement.
Vitamin D is a useful example. Individual studies have produced mixed results, and a 2026 systematic review and meta-analysis of randomized trials concluded that current randomized evidence does not show a clear, reproducible effect of vitamin D supplementation on total testosterone or androgen-bioavailability measures in adult men.
Treat documented deficiencies because the deficiencies themselves deserve appropriate treatment.
Do not assume a supplement can substitute for a hormonal diagnosis.
Do This Next: Address reversible contributors such as metabolic health, sleep, nutrition, recovery, and medication issues with the appropriate clinician, but do not use “lifestyle optimization” as a reason to ignore repeatedly abnormal laboratory findings.
Does Low Testosterone Mean You Need TRT?
No.
A low testosterone result does not automatically mean you need testosterone replacement therapy.
Clinicians may consider TRT when a patient has properly confirmed hypogonadism, symptoms worth treating, appropriate evaluation of the cause, fertility considerations have been addressed, and no contraindication makes therapy inappropriate.
That is different from:
“I want my testosterone higher.”
What benefits of TRT are reasonably supported?
Sexual function has some of the clearest evidence.
A systematic review of placebo-controlled trials found small average improvements in sexual desire, erectile function, and sexual satisfaction in hypogonadal men receiving TRT.
Depending on the patient and condition, treatment can also affect anemia, lean mass, and bone density.
What I would not promise is that TRT automatically produces:
- Endless energy
- Better cognition
- A cure for depression
- Large muscle gains
- A return to your 20s
TRT is a medical treatment with specific indications, potential benefits, limitations, and risks, not a general performance upgrade.
Does TRT affect fertility?
Yes.
This should be discussed before the first prescription.
Exogenous testosterone suppresses hypothalamic and pituitary signaling, reducing LH and FSH and potentially suppressing sperm production substantially.
The AUA/ASRM male-infertility guideline states that clinicians should not prescribe exogenous testosterone therapy to a man interested in current or future fertility.
Recovery after stopping testosterone may occur, but the timing varies and should not be treated as guaranteed.
If future children are even a possibility, say that clearly before treatment starts.
What side effects and risks can TRT involve?
Potential adverse effects and treatment issues include:
- Increased hematocrit or erythrocytosis
- Acne or oily skin
- Suppressed sperm production and fertility
- Increased blood pressure
- Formulation-specific reactions or complications
- The need for prostate-risk assessment and monitoring where appropriate
Erythrocytosis is one of the most consistently documented treatment effects. Endocrine Society guidance recommends measuring hematocrit at baseline, again after treatment begins, and periodically thereafter.
FDA's February 2025 class-wide labeling action also required or strengthened warnings about increased blood pressure after ambulatory blood-pressure studies found increases across testosterone products.
Is TRT safe for the heart?
The best answer is more reassuring than it was a decade ago and still more nuanced than “totally safe.”
The large randomized TRAVERSE trial enrolled more than 5,200 middle-aged and older men with hypogonadism and preexisting cardiovascular disease or elevated cardiovascular risk.
A major cardiovascular endpoint occurred in 7.0% of the testosterone group and 7.3% of the placebo group, meeting the study's criterion for cardiovascular noninferiority.
That is important reassurance about the primary heart-attack/stroke cardiovascular endpoint in the population and treatment strategy studied.
It is not proof that every testosterone regimen is risk-free.
TRAVERSE also found higher incidences of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group.
A related TRAVERSE fracture trial found clinical fractures in 3.50% of men assigned testosterone versus 2.46% assigned placebo.
The Endocrine Society's July 2026 statement specifically highlights the pulmonary-embolism and fracture findings and notes that important long-term safety questions remain, including prostate-cancer risk.
So the responsible conclusion is not:
“TRT is dangerous.”
And it is not:
“TRT has been proven completely safe.”
It is:
The major cardiovascular endpoint in TRAVERSE was reassuring, while other safety signals and unanswered long-term questions reinforce the need for appropriate patient selection and monitoring.
What changed in FDA testosterone labeling?
This is an area where dates matter.
On February 28, 2025, FDA announced class-wide testosterone-labeling changes after reviewing TRAVERSE and postmarket ambulatory blood-pressure studies.
The agency:
- Recommended removing language from the boxed warning about increased adverse cardiovascular outcomes
- Retained the age-related limitation-of-use language at that time
- Required or strengthened warnings about increased blood pressure
Then, on June 18, 2026, HHS announced that FDA was requesting further testosterone-label updates that would:
- Remove the limitation-of-use language concerning age-related hypogonadism
- Revise information related to prostate-cancer risk
- Revise warnings concerning benign prostatic hyperplasia
Those June 2026 changes were announced as requested labeling updates.
Product-specific prescribing information can be updated on different timelines, so current labeling for the actual medication should be checked rather than assuming every product label is identical.

What monitoring matters after TRT starts?
Good treatment does not end when the prescription is written.
Monitoring commonly includes:
- Whether symptoms actually improve
- Testosterone response
- Hematocrit
- Adverse effects
- Blood pressure
- Appropriate prostate monitoring
- Treatment adherence
- Formulation-specific issues
Endocrine Society guidance recommends reassessing clinical response and adverse effects after treatment starts, measuring testosterone after initiation, and monitoring hematocrit over time.
Ongoing follow-up is part of longitudinal medical care, not an optional add-on after the prescription.
What Should a Good Longevity or Men's Health Clinic Do Before Recommending TRT?
A good clinic should be willing to tell you that you do not need TRT.
That is one of the simplest ways to distinguish a diagnostic process from a treatment process that already knows its answer.
Green flags
I want to see a clinic that:
- Asks which symptoms actually changed
- Confirms low testosterone correctly
- Understands total testosterone, free testosterone, and SHBG
- Uses LH and FSH appropriately to investigate the cause
- Reviews medications and relevant health conditions
- Asks about fertility before treatment
- Evaluates sleep and metabolic health
- Explains realistic benefits without promising miracles
- Establishes baseline safety information
- Has a written follow-up and monitoring plan
- Is comfortable concluding that TRT is not indicated

Red flags
I would slow down when:
- TRT is recommended from one blood test
- The low result was never confirmed appropriately
- Nobody asks about fertility
- The clinic talks endlessly about “optimal” numbers but cannot explain clinical hypogonadism.
- Nearly every patient ends up on essentially the same protocol
- Nobody can explain why testosterone may be low
- Safety questions are dismissed instead of discussed
- The sales process seems more developed than the diagnostic process
If a clinic knows the treatment before it has confirmed the diagnosis, the process is backwards.
Which doctor treats low testosterone?
Several types of clinicians can play a role.
A primary-care clinician can begin the evaluation and look for common reversible contributors.
An endocrinologist is particularly useful when pituitary, hypothalamic, metabolic, or other complex endocrine disease is suspected.
A urologist, especially one focused on men's health or reproductive urology, can be especially useful when sexual function, testicular disease, or fertility is central.
A longevity or men's-health clinic can also provide good care when appropriately licensed clinicians follow evidence-based diagnostic, treatment, and monitoring standards.
The name on the door matters less than the quality of the clinical reasoning inside it.

Questions to ask before starting TRT
Ask:
- Why do you think I have hypogonadism rather than one low testosterone result?
- Should the testosterone measurement be repeated?
- Could my SHBG make free testosterone useful?
- What do my LH and FSH suggest about the cause?
- Could medications, metabolic health, illness, sleep, or nutrition be contributing?
- How could testosterone treatment affect my fertility?
- What are you checking before treatment for safety?
- What will you monitor after treatment starts?
- What would make you decide not to prescribe TRT?
That last question tells you a lot.
A serious clinician should have an answer.
Frequently Asked Questions About Low Testosterone
Does one low testosterone test mean I have low T?
No. Male hypogonadism generally requires compatible symptoms or signs plus consistently low testosterone confirmed with at least two appropriately collected morning tests. One abnormal result can be affected by biological variation, illness, medications, testing conditions, and laboratory method.
What time should testosterone be tested?
Current Endocrine Society guidance recommends early-morning, fasting testosterone testing. An initially low result should generally be confirmed on a different morning before diagnosing hypogonadism.
Can low testosterone be temporary?
Yes. Acute illness, medications such as opioids or corticosteroids, severe nutritional or energy restriction, obesity, and other reversible conditions can suppress testosterone or complicate its interpretation. That is one reason a diagnosis should not be based on one isolated result.
Is testosterone below 300 ng/dL always low?
No. Around 300 ng/dL is a common clinical threshold, but diagnosis still requires compatible symptoms or signs, accurate testing, repeat confirmation, and appropriate clinical interpretation.
Does low testosterone automatically mean I need TRT?
No. Treatment decisions should follow confirmation of hypogonadism, investigation of the cause, assessment of symptoms and potentially reversible contributors, and discussion of fertility, contraindications, benefits, and risks.
What do LH and FSH tell you when testosterone is low?
LH and FSH help show where the problem may originate. Elevated gonadotropins can support a primary testicular pattern, while low or inappropriately normal LH/FSH can point toward a secondary hypothalamic-pituitary pattern.
Total testosterone vs. free testosterone: which matters?
Total testosterone is usually the starting measurement. Free testosterone is especially useful when total T is borderline, or SHBG is abnormal and may be making the total result misleading.
What labs should I get before considering TRT?
Start by confirming morning total testosterone. Depending on the result and clinical context, the evaluation may then involve SHBG/free testosterone, LH, FSH, prolactin, iron studies, targeted cause-finding tests, fertility evaluation, and appropriate pretreatment safety assessments.
Does TRT affect fertility?
Yes. Exogenous testosterone suppresses the hormonal signals needed for normal sperm production and should not be prescribed to men interested in current or future fertility without appropriate reproductive counseling and an alternative treatment plan.
Is TRT safe for the heart?
TRAVERSE found that TRT was noninferior to placebo for major cardiovascular events in the high-risk hypogonadal men studied. However, atrial fibrillation, acute kidney injury, and pulmonary embolism occurred more often in the testosterone group, and a related trial found more clinical fractures. TRT should therefore not be described as universally risk-free.
What Should You Do Next If You Think Your Testosterone Is Low?
Do not start by trying to raise testosterone.
Start by trying to understand it.
1. Confirm it
Get appropriately timed morning testing and repeat an abnormal result.
If total testosterone is near the lower end of the range, determine whether SHBG and free testosterone change the interpretation.
2. Explain it
Ask why the number is low.
Is the pattern testicular?
Pituitary or hypothalamic?
Could obesity, medication, illness, severe energy deficit, previous anabolic-steroid exposure, or another reversible factor be contributing?
A diagnosis that never attempts to answer “why?” is incomplete.
3. Then decide what deserves treatment
If hypogonadism is confirmed and the cause, symptoms, fertility goals, contraindications, and risks support treatment, TRT may become a reasonable option to discuss with a qualified clinician.
If a reversible contributor deserves attention first, address it appropriately.
If the laboratory findings never establish hypogonadism, you may have avoided treating a condition you did not actually have.
The win is not getting your testosterone number as high as possible.
The win is understanding what the result means well enough to make a better decision.
